United Dental Resources Corporation and the AmericaSmiles Network (“UDRC”, “AmericaSmiles”, “us,” or “we,”) are committed to respecting the privacy rights of our customers, visitors, and other users of AmericaSmiles.com, FindACosmeticDentist.com and our network of on-line patient referral sites (the “Site” or “Sites”) and related websites, applications, and services provided by AmericaSmiles and in which this Privacy Policy is posted or referenced (collectively, the “Services”). This Privacy Policy applies only to AmericaSmiles Sites and Services. This Privacy Policy does not apply to any other website or digital service that you may be able to access through the Services or any website or digital services of AmericaSmiles’ business partners, each of which may have data collection, storage and use practices and policies that may differ materially from this Privacy Policy. Your use of the Services is governed by this Privacy Policy and our Terms and Conditions. Any capitalized term used but not defined in this Privacy Policy shall have the meaning in our Terms and Conditions.

2. Information We Collect
2.1 Personal Information
(a) Generally. Some of the Services require us to know who you are so that we can best meet your needs. When you access these Services, we may ask you to voluntarily provide us with certain information that personally identifies (or could be used to personally identify) you (“Personal Information”). You may still access and use some of the Services if you choose not to provide us with any Personal Information, but the features of the Services that require your Personal Information will not be accessible to you.

2.3 HIPAA and PHI
Under a federal law called the Health Insurance Portability and Accountability Act (“HIPAA”), some of the demographic, health and/or health-related information may be considered “protected health information” or “PHI”. AmericaSmiles does not collect such information and when AmericaSmiles receives identifiable information about you we will NOT retain it. . HIPAA provides specific protections for the privacy and security of PHI and restricts how PHI is used and disclosed. AmericaSmiles will NOT use nor disclose your PHI in any way or for any purpose.
3. How We Collect Information
We collect information, including Personal Information and Traffic Data, when you use and interact with the Services, such as:
When you use the Services’ interactive tools and services, such as searching for Healthcare Providers, When you voluntarily provide information in free-form text boxes through the Services and through responses to surveys, questionnaires and the like;
If you download and install certain applications and software we make available, we may receive and collect information transmitted from your computing device for the purpose of providing you the relevant Services, such as information that lets AmericaSmiles know when you are logged on and available to receive update or alert notices;
If you download our mobile application(s), we may receive information about your location and mobile device;
Through cookies, web beacons, website analytics services and other tracking technology; and
When you use the “Contact Us” function on the Site, send us an email or otherwise contact us.
We also may collect information about you from third party sources.
4. Our Commitment to Children’s Privacy
Protecting the privacy of the very young is especially important. For that reason, we never collect or maintain information at our website from those we actually know are under 13, and no part of our website is structured to attract anyone under 13. The Services are not intended for use by children and children are prohibited from using the Services.
5.Use of Information
We use the information you provide about yourself to provide the Services to you information you request and respond to correspondence that we receive from you; provide, maintain, administer or expand the Services, perform business analyses, or for other internal purposes to support, improve or enhance our business, the Services, and other products and services we offer;
We do not share this information with outside parties except to the extent necessary to complete an order you have placed.
You can register with our website if you would like to receive access to the registered-users section or and materials, as well as updates on our new products and services. Information you submit on our website will not be used for this purpose unless you fill out the registration form.
We use non-identifying and aggregate information to better design our website and may choose to share with advertisers in the future. For example, we may tell an advertiser that X number of individuals visited a certain area on our website, or that Y number of men and Z number of women filled out our registration form, but we would not disclose anything that could be used to identify those individuals.
Finally, we never use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses

7. Public Information
Any information that you may reveal in a review posting or online discussion or forum is intentionally open to the public and is not in any way private. We recommend that you carefully consider whether to disclose any Personal Information in any public posting or forum. What you have written may be seen and/or collected by third parties and may be used by others in ways we are unable to control or predict.

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online.However, we do not and cannot guarantee the security of your Personal Information. The effective date of this Privacy Policy is set forth at the top of this policy statement. We will notify you of any material change by posting notice on the websites where this policy is posted. Your continued use of the Services after the effective date constitutes your acceptance of the amended Privacy Policy. We encourage you to periodically review this page for the latest information on our privacy practices. The amended Privacy Policy supersedes all previous versions. IF YOU DO NOT AGREE TO FUTURE CHANGES TO THIS PRIVACY POLICY, YOU MUST STOP USING THE SERVICES AFTER THE EFFECTIVE DATE OF SUCH CHANGES.
